| Essential Job Functions and Responsibilities: The following statements are intended to describe the general nature and level of work being performed by individuals assigned to this position. They are not intended to be an exhaustive list of all duties, responsibilities and skills required of personnel so classified. · Assists with supervision of care as may be required for e-evaluation of the patient's continuing needs · Completes documentation in a manner · Documents patient services rendered and reports problems identified to the Supervising Nurse and proposes a plan of correction. · Ensures that the skilled nursing care per the patient's Plan of Care is satisfactorily and safely fulfilled · Identifies the need for other discipline involvement, consults with physician, obtains physician order and submits referral for same to the appropriate individual for coordination · Initiates preventative and rehabilitative nursing procedures for the patient's care and safety as appropriate · Participates in On-Call rotation as required · Participates in Patient Care Planning Conferences and revises the Plan of Care as changes occur · Performs a complete physical assessment of the patient and evaluates the home and the support family as is appropriate · Performs other duties as assigned · Reports observed patient changes and reactions to treatment to the physician and documents same in the daily visit note · Teaches, supervises and counsels patient and family members regarding patient care needs |
