Description:

Provides case management services to patient and their families in a multidisciplinary team approach. This is accomplished through assessing, planning and intervening with members of the patient's health care team, and evaluating the outcomes of care. Teaches pa¬tients and families about health care issues and discharge planning. Conducts Utilization Review to determine appropriate level of care. Uses an understanding of the principles of growth and development to assess each patient's age specific needs and provide age specific treatment and care. Qualifications:

RN with hospital med surg experience, previous case management experience, including utilization review preferred. BSN preferred.
